Democrat Dr. Amy Acton has won the Ohio gubernatorial election, defeating Republican biotech entrepreneur Vivek Ramaswamy in a major midterm upset that flips the state’s highest office to Democratic leadership for the first time in two decades.
The 60-year-old public health physician and former state health director will become Ohio’s first female governor, succeeding two-term Republican Governor Mike DeWine. Her victory comes despite extensive spending by Ramaswamy and his high-profile national backers, including a $5 million political action committee injection from Elon Musk.
Campaign Backlash and Controversial Attack Ads
The contest turned bitter in its final stretch as Ramaswamy faced intense bipartisan criticism over his campaign tactics:
- Controversial Ad Campaign: Ramaswamy drew severe national backlash for airing a televised advertisement featuring Acton’s estranged mother. The spot raised widespread public indignation after details revealed Acton’s mother had surrendered custody decades prior following her husband’s indictment for abusing Acton as a child.
- Primary Fractures: Ramaswamy’s entry into state politics caused internal friction within the Ohio Republican Party. Primary opponent Casey “The Car Guy” Putsch captured nearly 18% of the Republican primary vote before endorsing Acton in the general election.
National GOP Push Falls Short
Despite top-tier support from President Donald Trump and Vice President JD Vance—who made multiple campaign visits to Vance’s home state—Ramaswamy was unable to secure the conservative base.
Acton leveraged voter anxiety regarding high cost of living, gas prices, and tariff impacts, contrasting her public health background with Ramaswamy’s billionaire status. Her victory marks one of the biggest Democratic state-level gains in the 2026 midterm elections.
